KẾT QUẢ ĐIỀU TRỊ TỔN THƯƠNG SỤN VIỀN KHỚP VAI DO CHẤN THƯƠNG BẰNG PHẪU THUẬT NỘI SOI

Đặt vấn đề: Sụn viền làm diện tích tiếp xúc của ổ chảo với chỏm xương cánh tay tăng lên 75% theo chiều dọc và 57% theo chiều ngang. Tổn thương sụn viền làm giảm đáng kể lực ổn định vai. Mục tiêu nghiên cứu: Đánh giá kết quả điều trị tổn thương sụn viền khớp vai do chấn thương bằng phẫu thuật soi. Đối tượng và phương pháp nghiên cứu: Nghiên cứu tiến cứu, mô tả cắt ngang trên 41 bệnh nhân tổn thương sụn viền khớp vai do chấn thương, tất cả bệnh nhân được nội soi và cố định bằng chỉ neo. Kết quả: Thời gian theo dõi trung bình là 13 tháng. Điểm đau VAS giảm từ 4,46 còn 0,73 (P<0,001). Xoay ngoài và dạng vai tăng từ 37,56° lên 86,09° và từ 86,34° lên 163,91° (cả hai P<0,001). Tỷ lệ trở lại thể thao là 88,24%, tỷ lệ trở lại thể thao đạt mức trước chấn thương là 63,33%. Điểm Rowe và ASES tăng từ 45,61 lên 89,67 và từ 43,21 lên 92,1 (cả hai P<0,001). Chưa ghi nhận biến chứng nhiễm trùng hay trật khớp tái phát sau phẫu thuật. Kết luận: Phẫu thuật nội soi là phương pháp hiệu quả điều trị tổn thương sụn viền khớp vai do chấn thương.
