Quadrigeminal cistern lipoma

Small well defined lesion in the left paramedian region of the quadrigeminal cistern. It elicits a high signal on both T1 and T2, attenuated on fat sat sequences with blooming artifact on GE. No enhancement is seen on postcontrast sequences. A mild extension into the lower aspect of the aqueduct of Sylvius is noted with mild dilatation of the 3rd and lateral ventricles (hydrocephalus).

MRI features characteristic of a quadrigeminal cistern lipoma obstructing the aqueduct of Sylvius with mild hydrocephalus.
