Twiddler syndrome

Case contributed by Sebastian Röhrich
Diagnosis certain

Presentation

The patient was referred to check for a broken pacing lead.

Patient Data

Age: 70 years

Only the atrial lead is visible in a normal projection. The ventricular lead is seen curled up and projecting over the generator.

x-ray

A previous examination a few month ago showed both leads in a normal projection.

Case Discussion

The referring physician asked the patient who admitted to regularly fiddle around with the generator.

The leads were then replaced surgically and the extravascular position of the ventricle lead was confirmed during the procedure.
