Presentation
Persistent pain in the right knee, anteriorly, in a young volleyball player.
Patient Data
Fragmented lower pole of the patella and hyperechoic adjacent Hoffa's fat with cartilage swelling and slight proximal patellar tendon thickening. Remaining objectivity is normal.
Case Discussion
Sinding-Larsen-Johansson disease is an osteochondrosis affecting the inferior pole of the patella as a result of excessive traction of the patellar tendon on the insertion at the lower pole of the patella. Patellar sleeve fracture is bone avulsion at the proximal patellar tendon insertion along with underlying cartilaginous injury which can be seen on MRI, whereas Sinding Larsen Johanson disease is only bone avulsion at the proximal patellar insertion.
