Diffuse esophageal spasm

Uncoordinated peristalsis with simultaneous contraction is observed in the lower half esophagus causing a corkscrew appearance.
A small-sized hiatal hernia is also present.
Case Discussion
The corkscrew appearance in the esophagus, known as rosary bead esophagus, suggests diffuse/distal esophageal spasm (DOS) which is a motility disorder of the esophagus. It should reminded that only 60% of barium swallows will be abnormal, and <5% will show corkscrew esophagus.
