The three cases of old blowout fractures (BOFs) with symptomatic eye ache and eyestrain were reconstructed by endoscopic techniques. The surgical findings of the BOFs coincided with eye symptoms. After surgery, the eye symptoms were markedly decreased. Case 1: a 23 year-old female with medial type BOF complained of eye ache and eyestrain on lateral gaze. Three years after the eye injury, she had endoscopic endonasal surgery. The medial orbital wall of the BOF had adhered to the superior turbinate. Case 2 : a 46 year-old male continued to complain of diplopia and eye ache on lateral gaze 9 months after injury. The medial orbital wall of BOF was adhesive to the middle turbinate. Case 3 : In a 32 year-old male, the entrapment of the orbital content was observed in the inferior part of the ethmoid sinus, very close to the attachment of the 111 ground lamella. In this Case, a monopolar electric coagulator poses the possibility of a blackout.
