Abstract

A case of right obturator hernia with acute appendisitis preoperatively diagnosed is reported.
An 85-year-old woman was admitted to the hospital because of a right lower quadrant pain. She complained of right medial femoral pain while preoperative examination and was diagnosed as right obturator hernia with acute appendicitis by CT scan of the pelvic region. Appendectomy and hernioplasty were carried out. Postoperative course was uneventful. There has been no sign of recurrense for one year and two month.
Obturator hernias are reported in 387 cases in the Japanese literature from 1927 to 1994. When compared patients treated for a recent three years from 1992 to 1994 with those for 65 years before 1991, the rate of correct preoperative diagnosis has increased and the mortality lowered for CT scan of the pelvic region.
Obturator hernia commonly occurs in aged people. So we have to make early diagnosis by careful interview and preoperative examinations with subsequent operation while their general conditions are kept at a good level.
