A CASE REPORT OF ADENOACANTHOMA OF THE INTRA-THORACHIC ESOPHAGUS

Abstract

A 46-years-old man visited with the chief complaint of back pain and dysphagia. Upper gastrointestinal series and gastrointestinal fiberscope revealed a primary adenocarcinoma of the esophagus. Then he addmited to our hospital for the operation. Upper gastrointestinal series showed the tumor which was 11cm in size, circular type and well defined margine at the site of the middle esophagus. Chest tomography and computed tomography revealed a cancer invaded the left main bronchus and the wall of dessending aorta. The left thoracotomy was performed. The tumor was removed from the left main bronchus as much as possible. the dessending aorta was simultaneously resected with the esophagus by 11cm of length, and replaced with the graft. Cervical esophagocutaneostomy was made. The surgical disease staging was A3N3M0Pl0 and Staghe IV. Macroscopically, the tumor mass was 8.5×4.0cm in size with well defined marging and prominent type, and involved the aorta. Microscopically, the tumor was moderately differenciated adenocarcinoma with a part of squamous cell carcinoma pattern. The pathological diagnosis was adenoacanthoma, IFNγ, ie(+), ly(+) and v(+), involving the outward wall of the aorta. Six months after operation, he died of the recurrence and the autopsy was made.
