Abstract

O objetivo deste estudo foi avaliar se a Síndrome da Apneia Obstrutiva do Sono de indivíduos com discrepância esquelética de Classe II, apresenta melhora após a cirurgia ortognática de avanço bimaxilar. Esta revisão sistemática foi realizada através da busca eletrônica nas bases de dados MEDLINE (PubMed) e Scopus (Elsevier). Estudos que avaliaram variáveis pré e pós-operatórias relacionadas a SAOS e via aérea foram incluídos. Os dados coletados e analisados foram: características demográficas; polissonografia pré e pós-operatórias; índice de massa corporal; análise subjetiva de sonolência; cirurgia realizada; tempo de acompanhamento pós-operatório; análise da via aérea; reintervenção; cirurgias prévias envolvendo via aérea. A qualidade dos estudos foi avaliada utilizando a Escala de Hawker. A busca inicial englobou 155 artigos e ao final 5 foram incluídos. No total 54 pacientes foram submetidos a cirurgia ortognática, sendo a maioria (90,7%) homens com idade média de 43,7 anos. A média do IMC foi de 34,05 kg/m2. O tempo de acompanhamento pós-operatório variou de 2 a 6 meses. Em nenhum dos pacientes da amostra foi necessária a reintervenção. De acordo com os parâmetros observados nos exames de polissonografia apresentados nos estudos selecionados nesta revisão sistemática, pode-se concluir que o avanço mandibular combinado ou não com a cirurgia maxilar ou de mento, contribui para uma melhora do valor de IAH e por conseguinte, com a análise subjetiva do sono e com o quadro de SAOS.
