　ソフトコンタクトレンズ（SCL）装用によるgiant papillary conjunctivitis（GPC）の2症例を経験し，その生検組織を電子顕微鏡により観察した。症例は共に約6年間のSCL使用経験を有する22歳と25歳の女性で，眼部掻痒感を伴い上眼瞼結膜全体に乳頭の増殖を認めた。組織所見は上皮層では上皮の肥厚とリンパ球，好酸球，肥満細胞，ランゲルハンス細胞（ラ細胞）などの浸潤を認めた。固有層では形質細胞，リンパ球および多数の肥満細胞と好酸球の浸潤を認め，特に肥満細胞の多くは脱顆粒像を呈していた。さらに従来報告されていないラ細胞が多数観察され，リンパ球や形質細胞などと近接している像が多く観察された。これらの組織所見は春季カタルにみられる組織像とほぼ一致しており，GPCがアレルギー性反応に基づく病変であることが示唆された。またラ細胞も体病の免疫病理に何らかの関与をしているものと考えられた。

We examined the histopathylogy and ultrastruc-ture of giant papillary conjunctivitis associated with soft contact lens wear in two cases. Both cases had been habitual wearers of soft contact lens for the past 6 years.

In the epithelium of the upper tarsal conjunc-tiva, there were eosinophils, mast cells, lymphocy-tes and Langerhans cells in both cases. An increased number of plasma cells, lymphocytes, mast cells, eosinophils and Langerhans cells were seen in the substantia propria. Mast cells showed various stages of degranulation.
