緒　言

　螢光眼底造影法による糖尿病性網膜症の基本的な病態の解析は，所見の客観的記録を行ないやすい後極部については，多くの研究があるが，周辺部をも含めた眼底全体における糖尿病性網膜症の病像は，極めて不完全にしか把握されていない。糖尿病性網膜症の視力の最終予後を最も左右する硝子体出血および増殖性変化には，新生血管の有無が大きく関与しており，新生血管が乳頭は別として，眼底周辺部に好発すること，ならびに，近年提唱されている光凝固手技のひとつpanretinalphotocoagulationが全眼底を対象とすることからも，糖尿病性網膜症における眼底周辺部の関与は重要な研究課題である。われわれは，独自に開発した高解像力・広画角のパノラマ螢光眼底造影の手技を本症の多数例に応用検索することにより，準周辺部眼底での網膜血管病変が，重症糖尿病性網膜症での基本的な変化であること，ならびに，これが新生血管の出現と密接な関連をもつことを知つた。本知見は良性網膜症と悪性網膜症とをつなぐ重要な鎖の一環をなすものと考えられ，以下にその大要を報告する。

We examined and analysed 74 eyes with diabet-ic retinopathy of various degrees by means of a newly developed panoramic fluorescein angio-graphy technique. This technique enabled the documentation of fluorescein angiographic find-ings over a fundus area of up to 130 degrees in diameter. Usually, the midperipheral fundus zone outer to the area of distribution of radial peri-papillary capillaries (RPC's) was found to be more extensively and intensely affected in the form of occlusion of retinal vascular bed and preretinal vasoproliferation than in the posterior fundus area.
