Ⅰ．緒言

　頭部外傷の直後に検眼鏡的に殆んど異常所見が認められないにも拘らず片眼に著明な視力障害を招く事が屡々ある。諸家の統計的報告によればTurnerは1.5％Phelbsは2.4％Landoltは0.8％Braendleは0.75％Wankeは1.7％から5.2％の間であるとしている。その原因は障害がむしろ眼球内に存在するのでなくて，球後の視神経で頭蓋内の視交叉に到る迄の処にある事は今日広く知られている処である。そしてこの原因や治療法については最近十年間に於て多くの研究，臨床例，手術例の報告がなされてきた。その結果現在云われる処は頭部外傷直後に起つた略々失明に近い症例に対してはその原因の如何に拘らず手術はさし控える方が良いという結論である。著者等は最近三ヵ年に渉つて仁保氏^2）等の方法を元として経副鼻腔視束管開放手術27例行い，現在の諸家によつて略々一致した結論が或程度訂正されねばならぬ色々の症例を経験し又頭部外傷による視力障害の原因に就いても些か考察を試みる事も出来る様になつた。これを報告し大方の御批判を仰ぎたい。

A report is presented over 27 cases of po-sttraumatic impairment of vision treated by surgical decompression of the optic nerve through the transethmoidal route during the 3-year period of 1962-64. No serious compli-cation resulted in all of the operated cases. When improvement in visual acuity was ta-ken as criterium for the evaluation of outco-mes of the operation, the result was judged as excellent in 7 cases, as fair in 13 and indifferent in 7 cases. In no instance was there further deterioration of vision due to the operation.

A detailed description of the surgical method is presented.
