　桐沢型ぶどう膜炎8例8眼に硝子体手術を行った。男性7例，女性1例で，年齢は30～80歳，平均63歳である。全例に水痘・帯状疱疹ウイルスの関与があった。手術は，アシクロビル・副腎皮質ステロイドを投与した後に，網膜剝離が発症するか硝子体混濁が増強した場合に限った。輪状締結，水晶体乳化吸引，眼内レンズ挿入を適宜併用したが，薬物の眼内灌流は行わなかった。最終視力は，網膜剝離発症後に手術を行った4眼中3眼では0.5以上，発症前に手術を行った4眼中1眼では0.1，3眼では0.01以下であった。初回手術でシリコーンオイルタンポナーデを6眼に行い，SF₆ガスを用いた2眼は術後網膜剝離になり，再手術でシリコーンオイルタンポナーデを行った。網膜剝離の有無にかかわらず，滲出病変が後極部に波及しなかった場合の術後視力は比較的良好であった。

　We performed vitreous surgery on 8 eyes of 8 patients of acute retinal necrosis. The series comprised 7males and 1 female. Their ages ranged from 30 to 80 years,averaged 63 years. All the cases were positive for varicella-zoster virus. Vitreous surgery was performed only after onset of retinal detachment or increased vitreous opacity. We occasionally performed scleral encircling,phacoemulsification-aspiration,or intraocular lens implantation,but no irrigation of the vitreous with medications. Out of 4 eyes operated after onset of retinal detachment,3 eyes resulted in the final visual acuity of 0.5 or over. Out of the other 4 eyes operated before onset of retinal detachment,one eye resulted in final visual acuity of 0.1 and 3 eyes in 0.01 or less. Silicone oil tamponade was used in 6 eyes during the initial surgery. The other 2 eyes developed retinal detachment after receiving SF₆tamponade,necessitating silicone oil tamponade. The findings show that the final visual acuity was fair if the posterior fundus was not involved by exudation irrespective of the presence of retinal detachment.
