　肺疾患に基づく肺高血圧症は右室に負荷をおよぼし，究極的には肺性心に到ることも多い。しかしながら，その原因は種々雑多であり、右室負荷の程度も基礎疾患により異なる。

　なかでも原発性肺高血圧症（PPH）^1,2）は全く原因不明の稀な難治性疾患で肺血管病変を主体とし，直接右室へ影響をおよぼすために右室圧負荷の代表的疾患の1つである。一方．慢性肺気腫（CPE）^3）は肺実質病変であり肺胞低換気による種々の影響の結果，肺循環血液量の増加を生ずるため，右室への容量負荷疾患の1つとされる。

In order to clarify the factors influencing pulmonary hypertension and eventually right ventricular failure in patients with primary pulmonary hypertension (PPH) and chronic pulmonary emphysema (CPE) wete studied from the standpoint of pressure and volume overload.

The following conclusion were elicited.

1) Pulmonary at terial, right ventricular pressure, total pulmonary vascular resistance and right ventricular work index are significantly higher (p<0.001) in patients with PPH. No differences are noted in right ventricular enddiastolic pressure and cardiac index.
