Veränderungsmessung und Ergebnismonitoring in der klinischen Routine
Abstract
Veränderungsmessung und Ergebnismonitoring sind nützliche Werkzeuge zur Stärkung der Gesundheitsversorgung. Sie zielen auf die Sicherung bzw. Verbesserung der Behandlungsqualität und die Erweiterung der Behandlungsoptionen. Im Rahmen klassischer Versorgungskonzepte haben besonders die Behandlungsplanung sowie die Identifikation von nicht erfolgreichen Behandlungen bzw. Nebenwirkungen das Potential, Verbesserungen zu fördern. Die Herausforderung besteht in der Interpretation der Daten und dem Ziehen der angemessenen Schlussfolgerungen. Die Konzepte der reliablen und der klinisch signifikanten Veränderung haben sich bei der Beschreibung von Behandlungs- und Gesundungsverlauf im Einzelfall bewährt, beispielsweise im Rahmen des Stuttgart-Heidelberger Modells der Qualitätssicherung. Neue Anwendungsmöglichkeiten für ein effizientes einzelfallbezogenes Ergebnismonitoring über die Qualitätssicherung hinaus eröffnen sich über die moderne Informations- und Kommunikationstechnologie mit der praktischen Umsetzung innovativer, adaptiver Strategien zur Verbesserung der Versorgung für Patienten.
Measurement of change and outcome monitoring are useful tools enhancing mental health services. Their use aims at assuring the quality of treatment and extending treatment options. Optimizing treatment planning, the identification of negative developments and ineffective treatment episodes have a considerable potential to improve mental health services. However, interpretations of data describing change and outcome remain a challenge, especially on individual level. Concepts of reliable and clinically significant change have been introduced for this purpose. Their usability in clinical practice is exemplified within the Stuttgart-Heidelberg model of quality assurance. Beyond quality assurance, efficient measurement of change and outcome open new avenues for the development and implementation of innovative, adaptive treatment strategies to improve routine mental health care for individual patients.
