臨終癌症病患家庭之關懷

Caring for Cancer Patients and Family at the End of Life

摘要


疾病是一個家庭的事,家庭在癌症病人的臨終照顧扮演著很重要的角色,家庭的支持決定癌症臨終病人的生活品質,癌症也影響家庭成員的發展與健康,護理人員不但要照顧病人,也應該照顧其整個家庭。本文主要目的是藉由整合文獻查證,闡述能協助病人及家庭減輕痛苦並增進其療癒的一些照護措施。從電腦資料庫MEDLINE、CINAHL、PsycInfo之電子期刊,使用「癌症病患」、「家庭」、「生命末期」、「臨終」及「關懷」等關鍵字查詢適合的文獻,加以綜整並確認符合家庭及臨床實務需求的相關照護措施。結果發現能幫助癌末病患及家庭療癒情緒與減輕痛苦的措施有:陪伴、增進與病人及家屬的溝通、增進家人與臨終病人道別、滿足臨終家庭的文化需求。分別加以闡述這些措施的定義與實務應用。護理人員位於獨特的地位並具有特權能陪伴家屬,提供病人及家屬臨終關懷、支持與照顧,護理人員與家庭應建立良好的信任關係,敞開及信任的溝通,提供身、心、靈、社會與情緒的支持,並尊重家庭的文化需求,以增進整個家庭在經歷家人死亡過程的療癒與健康。期望藉由本文之文獻探討能提醒護理人員落實這些有益於臨終病人及家屬療癒的相關措施,並協助癌症病人及家庭度過生命的黃金時刻。

Cancer illness is a family affair. Family plays a very important role in caring for the cancer patients at the end of life. Family support is one of the most crucial determinants for quality of life of dying patients. In addition to the care of cancer patients, nurses need to incorporate their families into nursing caregiving. The purpose of this review was to identify some of the interventions that could soften cancer patients and families' suffering and enhance their healing at the end of life based on literature. Computerized databases MEDLINE, CINAHL, PsycInfo, and Amazon were used to identify the effective interventions. Key words ”dying”, ”family”, ”end-of-life”, ”caring” and ”healing” were used to guide the search of the literature. Some specific interventions that can help families who experiencing the dying process are: being present, enhancing best communication with the family, facilitating the family to say goodbye at the end of life, and responding to the cultural needs of the family. The definition and the practical application of these interventions were discussed. Nurses are in unique position to help families manage their lives when a loved one has terminal cancer illness. Providing end-of-life care during the golden moment is a privilege that should not be taken lightly. Open and trusting communication, physical, psychological, spiritual, social, and emotional support, and respect for families' cultural needs are important for promoting health and facilitating growth at the end of life. It is expected that the literature review can remind nurses at the oncology wards implementing these interventions that can help cancer patients and families at the end of life heal their emotion and promote their health, and go through the golden moment of life.
