長時間監測的無線光學式鼻量計

Wireless optical rhinometry for long time monitoring

摘要


此研究目的是發展一套可以長時間監測與記錄鼻腔黏膜充血量變化的無線光學式鼻量計。過敏性鼻炎患者受到過敏原刺激時會引發鼻腔黏膜與鼻甲的充血腫脹,嚴重時會造成鼻塞,影響呼吸的順暢性,而冷刺激型的鼻塞也常發生於早晨剛睡醒時。為了進一步瞭解在睡醒前後鼻腔黏膜的充血量變化,此研究已研製一套在睡眠時可長時間偵測記錄的無線光學式鼻量計。 此研究裝置藉由量測近紅外光穿透鼻腔的光亮度變化來檢測出鼻腔黏膜血液灌流量的改變,同時也使用熱敏電阻測量兩個鼻孔呼吸氣流量變化以確認是否鼻塞,並使用加速規來量測頭部的位移變化以瞭解側臥對兩側充血量的影響,由微處理機將這三種訊號經由藍芽模組傳輸至到智慧型手機並將所接收的資料儲存在記憶卡中,最後再將手機中的資料傳輸至電腦做訊號分析。 根據體位變換實驗、吸呼實驗與睡眠監測等實驗結果可以證明,頭部的位置變換從平躺到側臥時鼻腔的血液量會隨著重力而改變,同時也隨著吸呼的深淺而變化。整夜的睡眠監測實驗結果也證實此系統可以在不影響睡眠情況下同時測量到鼻道氣流、鼻腔黏膜血量變化與頭部的角度位置,這些資訊將有助於與鼻炎相關研究工作。

The purpose of this study is to develop a wireless optical rhinometer for long term monitoring of the change in nasal mucosal perfusion. Allergen stimulation will lead to swelling of nasal mucosa and turbinate in patients with allergic rhinitis, and even cause nasal congestion. Cold induced nasal congestion often occurs in the morning right after awakening. To further study the changes in the nasal mucosa perfusion before and after waking up, a wearable wireless optical rhinometer was built for long term recording. This device measures the transmission of near-infrared through nasal bridge as an index of nasal mucosal blood perfusion. It also measures the respiratory air flow by putting thermistors at nostrils in order to confirm nasal congestion. An accelerometer was used to measure the head position during sleep to understand how it affects congestion, A microprocessor acquires these signals and transmits them to a smartphone through Bluetooth. The received data was stored in the memory card for further procession in a computer. The results of postural experiment, breathing experiment and sleep monitoring experiment showed that nasal perfusion change, with head position because of gravity. It also affected by the depth of breathing. The whole night recording during sleep confirmed that the system can successfully record all the signals without affecting sleep. This device will be helpful in future study of rhinitis.
