協助一位乳癌術後合併淋巴水腫病患手臂復健運動
Nursing Care of a Post-mastectomy Patient with Lymphedema
摘要
本文是在描述護理人員如何協助一位罹患乳癌術後接受化學、放射治療之病患，因出現患側手臂淋巴水腫、關節活動度變差，於復健科門診接受復健治療，重建心理、身體和社會功能之護理經驗。於此個案門診復健過程中，筆者運用羅氏適應模式，以會談、觀察、身體評估、查閱病歷、電訪等方式收集資料，進行整體性的評估。確立個案之護理問題有：身體心像紊亂、身體活動功能障礙、知識缺失等三項之健康問題，在照護過程中，筆者主動關懷、傾聽並接納病患心中感受，協助個案克服罹患乳癌所帶來之不適與衝擊，並增進對乳癌術後合併淋巴水腫的了解，提供支持性的照顧及對疾病的適應，使其能更積極接受復健治療，在短時間內儘可能恢復其身體功能及自我照顧能力，重返家庭、社會，進而提升其生活品質及生命價值。在我國乳癌發生率逐年上升且年輕化的趨勢中，僅藉此個案護理經驗之分享，提供護理人員日後照顧乳癌術後合併淋巴水腫病患之參考。

This article describes the application of Roy's adaptation model in the nursing care of a post-mastectomy female rebuilding her bio-physical-social functions. The patient developed lymphedema on upper extremity of the breast cancer side after her radical mastectomy, chemotherapy and radiotherapy. Nursing assessment identified the patient's problems, which included disturbance in body image, mobility impairment and knowledge deficit. Use of active caring, acceptance and listening to the patient's feelings assisted the patient's coping and adaptation to the disease and treatment complications. The patient demonstrated active participation in arm exercise rehabilitation to relieve lymphedema. She was able to demonstrate self-care activities and discharged to home with an improved perspective and quality of life.
