本文為探討初罹患乳癌且接受乳房局部切除中年未婚女性,經由主治醫師轉介至癌症資源中心,結合護理師、社工師、病友志工與牧靈人員等團隊資源所提供的持續性照護經驗。照顧期間為2016年2月5日至7月25日,過程中運用Gordon十一項功能性健康型態作為評估工具,發現個案有焦慮、潛在危險性感染及身體心像改變的健康問題。針對這些健康問題癌症資源中心藉由持續關懷陪伴減緩心理衝擊,提供正確資訊與護理指導及合適資源且連結病友志工、病友團體,指導個案居家傷口照護及放鬆技巧,使其能以正向的態度面對疾病與接受治療。期望經由此護理經驗之分享,提供護理人員日後照護此類個案之參考。
This study explored the continuous nursing care provided by a team of nurses, social workers, volunteers, and chaplains at a cancer resource center. The patient was a single, middle-aged woman who underwent partial mastectomy for first-time breast cancer and was referred to the cancer resource center by an attending doctor. The nursing period was between February 5, 2016 and July 25, 2016. Gordon's 11 functional health patterns was employed as an assessment tool. During the nursing period, the patient faced anxiety, risk of infection, and other health problems that had a profound physical and mental influence. In response, the cancer resource center provided continuous care to mitigate any psychological impact by providing appropriate information and care guidance as well as by connecting the patient with volunteer groups. In addition, the center trained the patient on how to treat her wound at home and taught her relaxation techniques. These actions were taken to help the patient face her illness while undergoing treatment with a positive attitude. The results of this study can serve as a reference for nursing teams with similar patients.
