본 연구는 산후우울 성향 모의 심리사회적 경험을 양적연구와 질적연구 방법으로 조사하고 일반군과 비교하여 그 특성을 살펴보고자 하였다. 연구대상은 연구를 위해 종합병원 4곳의 산부인과에서 진행 중인 아동 아토피 천식 질환 원인 규명 코호트 연구 대상 중 본 연구에 참여중인 대상으로 실험군은 EPDS 13점 이상 모 8명, 일반군은 4점 이하 모 8명을 대상으로 진행하였다. 연구결과 성장배경, 배우자와의 관계, 자기인식, 양육자로서의 경험, 지원희망 분야 최종 5개의 영역이 구별되었다. 실험군은 성장배경에서 일반군에 비해 중립적이거나 부정적인 분위기로 보고했으며, 원가족의 의사소통 문제가 현재 가족의 의사소통에 부정적 영향을 주고 있는 것으로 나타났다. 자녀출산 시 배우자의 반응은 실험군이 일반군에 비해 부정적인 것으로 나타났으며, 산모 자신의 반응은 실험군이 일반군에 비해 덜 긍정적인 것으로 나타나는 등 산후우울 유무에 따라 다른 인식을 나타났다. 이 결과를 통해 추후 개발 될 산후우울 어머니의 심리적 개입프로그램에는 원 가족과의 관계표상(애착관계)과 영향력에 대한 부정적인 인지의 고리 차단하기와 현재 부부간의 자녀출산에 관한 부정적 경험과 관련된 요인을 다루는 개입 목표가 제안된 연구가 필요할 것으로 보인다.
The purpose of this study was to investigate the psychosocial characteristics of mothers with depression at 6-7 months postpartum. The subjects were assigned into an experimental group (n = 8; EPDS score ≧ 13; mean (±S.D.) = 16.8±3.4) and a control group (n = 8; EPDS score ≦ 4; 2.8±1.2). Five categories were extracted from a qualitative study: experience of being nurtured in the family-of-origin, feedback from the spouse related to pregnancy and delivery, self-awareness of postpartum depression, experience of being a fosterer, and a description of social support. The results showed that those in the experimental group were likely to report the mood of the family of origin as neutral or negative compared to those in the control group and that communication problems in the family of origin had a negative effect on communication in the current family. The responses of husbands regarding delivery were more negative, and those of mothers were less positive in the experimental group compared to the control group, indicating that there were differences caused by the presence of postpartum depression. It is recommended that future studies, based on these results, develop an intervention program for mothers with postpartum depression that blocks the negative perception linkage of relation-representation and the influence of the family of origin and addresses factors related to negative experiences pertaining to childbirth.
