보훈의료 대상자별 의료이용 특성을 반영한 의료비 산정체계 개발

Optimal Forecasting Models of Veterans’ Medical Expenses

보훈의료 대상자는 다른 보험가입자에 비해 고령인구 비율이 높으며 70세 이상이 전체의 60.2%를 차지하고 있다. 이에 정부는 보훈의료비 예산을 지속적으로 증액하고 있으나, 참전보훈 대상자의 급감, 중장기복무 제대자의 증가 등으로 보훈의료비의 예측정확도가 떨어지고 있다. 본연구의 목적은 보훈예산의 효율적 운영과 장기예측을 위해 보훈의료 대상자별 의료이용 특성을 반영한 의료비 산정체계를 개발하는 것이다. 연구방법으로는 추세분석모형, 이동평균모형, 기하 평균모형을 사용하였고, 보훈의료비 예산총액모형과 연인원, 진료단가의 세부항목으로 구성된 추계모형을 개발하였다. 추계모형의 예측정확도는 평균절대오차율(MAPE)로 판별하였고, 그 중 정확도가 가장 높은 모형을 산정체계로 선정하였다. 자료는 2007∼2018년 국가보훈처의 보훈의료비 예결산 자료를 이용하였다. 분석결과, 시계열자료를 토대로 분석한 추세분석모형의 예측정확 도가 가장 높게 나타났고, 기하평균모형과 이동평균모형은 상대적으로 낮게 나타났다. 이러한 결과는 보훈의료비 지원제도와 대상자 구분 정책의 큰 변동, 지불제도의 특성 때문에 나타난 것으로 판단되므로 안정적인 보훈의료비 지원제도와 감면제도의 확립, 기존의 행위별수가제에서 총액제로 지불제도를 변경하는 제도개선방안을 제안하였다.

The veterans are considered to have the highest elderly population among the health insurances in South Korea. The government continues to increase the veterans medical expense budget. However, the forecasting accuracy of veterans medical expenses is declining due to an abrupt decrease in the number of war related veterans and an increase in the number of long-term service veterans. The purpose of this study is to develop a medical expense estimation system that reflects the special characteristics of the veterans. Trend analysis model, moving average model, and geometric average model were used as forecasting models. Estimation methods for each budget item were developed by applying the total amount estimation and component estimation. The estimation accuracy was calculated by mean absolute percentage error. The model with the highest accuracy was selected as the calculation system. As a result, the trend analysis model showed the highest estimation accuracy. Factors that decrease the forecast accuracy include the frequent changes in the veterans’ medical expense system, patients classification policy, and payment system. This study suggests to establish a stable system of veterans’ medical expenses and to change the payment system from fee-for service to global budget system to improve veterans’ medical expenses estimation accuracy.
