Olanzapin je zástupce antipsychotik druhé generace, který je doporučován jako jedno z léčiv první volby u prvních epizod schizofrenie. Přes výrazné pokroky v léčbě psychotických poruch je dlouhodobé remise dosahováno přibližně jen u 30 % pacientů. Nejen proto se neustále hledají způsoby, jak zvýšit úspěšnost léčby a zároveň predikovat odpověď na konkrétní léčivo, s cílem tuto léčbu maximálně personalisovat a vybrat vždy takové léčivo, u nějž má konkrétní pacient nejvyšší šanci dosáhnout remise. Tento článek podává přehled genetických a epigenetických faktorů, které mohou ovlivnit farmakokinetiku olanzapinu (polymorfismy biotransformačních enzymů a transportních proteinů), a dále také přehled faktorů, které mohou ovlivnit léčebné cíle (polymorfismy genů pro 5HT2A, 5HT2C, D2, D3 receptory, úroveň acetylace histonů a demethylaci DNA).
Olanzapine as one of the second-generation antipsychotics, is recommended as afirst-line treatment for the first episodes of schizophrenia. Despite significant progress in the treatment of psychotic disorders, long-term remission is achieved in only 30% of patients. Due to this, there are investigated various ways to increase the success rate of the treatment and ways to predict the response of particular treatment with the aim of personalized treatment - i.e. selection of the drug with the greatest probability of remission in distinct patient. This article provides an overview of the genetic and epigenetic factors that affect pharmacokinetics of olanzapine (polymorphisms of biotransformation enzymes and transport proteins), as well as an overview of factors that affect therapeutic targets (5HT2A, 5HT2C, D2, D3 receptor polymorphisms, histone acetylation and DNA demethylation).
