Post-traumatic aphakia without capsular support can require a complex surgery and nowadays there are different techniques for the secondary correction with implant of IOL. We present the case of an implantation of sutureless scleral suspension intraocular lens (IOL) in a 42-year-old man with post-traumatic aphakia. The novelty lies in the use of a single-piece acrylic foldable IOL with scleral plugs at the ends that ensure anchoring to scleral tissue without the use of sutures. The IOL implantation was performed with no intraoperative complications. After surgery, visual acuity BCVA was significantly increased, the IOL showed a stable and centered position without any tilt or complication during the follow-up. Management of post-traumatic aphakia by sutureless scleral suspension IOL appears to be a promising approach which gives the surgeon the possibility to correct a complex case, which could be easier to perform and faster than other techniques; existence of foldable IOL also permits relatively small corneal incisions.
