Twenty-seven patients with facial acne vulgaris were topically received with an ointment containing 0.5% a triensteroid of antiandrogenic compound and significant improvement was recognized in 77% of the patients. The replacement sum of skin surface lipid was determined by cup method for each patient. The replacement sum in acne patients was obviously increased in comparison with that of normal persons and widely varied among the patients. Only a slight decrease in replacement sum was observed during the application of this compound, but no correlation between application duration and the level of surface lipid was seen. Thus, it is considered that this antiandrogenic compound improves acne vulgaris through suppression in hyperplasia and dilatation of sebaceous gland and consequent smoothing the sebum flow, not direct suppression of sebum secretion.
