열이 있는 3세 이하 환자의 심각한 세균감염 예측: 개별 염증표지자, Laboratory-score 및 새로운 염증표지자 결합모형의 비교

Abstract

Purpose

To compare the efficacy of inflammatory markers, the Laboratory-score, and a new laboratory combined model for predicting serious bacterial infection (SBI) in young febrile children.

Methods

The presence of SBI was reviewed in previously healthy children aged 3 years or younger with fever (> 38℃) who visited the emergency department from 2017 through 2018. Areas under the curves (AUCs) of the receiver operating characteristic curve for SBI were compared with individual inflammatory markers (white blood cells [WBC] count, erythrocyte sedimentation rate [ESR], C-reactive protein [CRP], procalcitonin [PCT], and urine WBC count), the Laboratory-score, and a laboratory combined model. The latter model was developed using logistic regression analysis including ESR, CRP, and PCT.

Results

Of the 203 enrolled children, SBI was diagnosed in 58 (28.6%). For SBI prediction, the Laboratory-score showed 51.7% sensitivity (95% confidence interval [CI], 38.2%-65.0%) and 83.5% specificity (95% CI, 76.4%-89.1%). The AUC of the Laboratory-score (0.76) was significantly superior to the values of all individual inflammatory markers (WBC, 0.59 [P = 0.032]; ESR, 0.69; and CRP, 0.74 [P < 0.001]) except that of PCT (0.77, [P < 0.001]). The AUC of the laboratory combined model (0.80) was superior to that of the Laboratory-score (0.76) (P < 0.001).

Conclusion

In this study, the new laboratory combined model showed good predictability for SBI. This finding suggests the usefulness of combining ESR, CRP, and PCT in predicting SBI.

서론

소아에서 국소증상 없는 열 환자는 의사소통 능력이 미숙하고 비특이적 증상을 보이므로, 심각한 세균감염(serious bacterial infection, SBI) 예측이 어렵다. 실제로 응급실을 방문한 열 환자에서 SBI의 빈도는 7%, 침습성 SBI는 5세 미만 환자에서 0.4% 미만으로[1], SBI 빈도 감소에도 불구하고 영유아 입원율이 증가하고 있다[2]. 따라서 객관적인 SBI 예측도구가 응급실 입원결정에 유용하다. 백혈구 수, C-반응단백질, 프로칼시토닌의 민감도 및 특이도는 백혈구 수 > 1,5000/㎣에서 53% 및 73%, C-반응단백질 4 ㎎/dL에서 81% 및 76%, 프로칼시토닌 0.5 μg/L에서 94% 및 68%로 각각 알려졌다[3]. Laboratory-score는 C-반응단백질 및 프로칼시토닌, 소변 백혈구 수 및 아질산염 양성 여부를 기준으로 SBI 발생을 예측하는 점수체계이다(Appendix 1). 이를 통한 SBI의 민감도 및 특이도는 94% 및 81%로 보고됐으며[3], 36개월 이하의 국소증상 없는 열 환아에서 SBI에 대한 민감도 및 특이도가 각각 59.2%-59.8% 및 84%-95%로 보고됐다[4,5]. 그러나, 한국에서는 관련 연구가 부족한 실정이다.
이에 본 연구진은 SBI 발생을 예측하기 위해 염증표지자 결합모형(Laboratory-combined model)을 제안했으며, 이는 임상에서 자주 사용하는 염증표지자인 적혈구침강속도, C-반응단백질 및 프로칼시토닌을 통해 계산한 예측모형이다. 본 연구의 목적은 열을 주소로 단일 기관 응급실을 방문한 3세 이하 환자에서 개별 염증표지자와 Laboratory-score, 염증표지자 결합모형의 SBI 예측성적을 비교하는 것이다.

대상과 방법

1. 연구대상

2017년 1월부터 2018년 12월까지 경기도 내 위치한 단일 권역응급의료센터 소아응급실을 방문한 36개월 이하 열(고막 체온 ≥ 38℃) 환자를 연구대상자로 정의했다. 이들 중 (1) 방문 전 48시간 이내 항생제요법, (2) 2일 이내 예방접종, (3) 면역약화상태, (4) 만성질환, (5) 열 5일 이상 지속, (6) 국소감염은 연구대상에서 제외했다. 본 연구는 본원 임상연구심의위원회의 승인을 얻은 후 시행했다(IRB No. UC19RESI0036).
임상 결과가 SBI 소견을 보이면 소아응급실 임상의는 입원 및 경험적 항생제요법을 시작했다. 중환자실 및 일반병실 입원은 소아응급실 전담전문의가 결정했다. 중환자실 입원 기준은 패혈쇼크, 기계환기 시행, 호흡수의 유의한 증가, 동맥혈산소분압 감소, 단순흉부방사선검사에서 심한 침윤, 의식 변화 및 경구섭취 가능 여부였다.

2. 자료수집

의무기록을 확인하여 환자의 나이(월) 및 성별, 열 지속기간(일), 활력징후(심장박동수, 호흡수, 체온), 백혈구 수, 적혈구침강속도, C-반응단백질, 프로칼시토닌 소변 백혈구 수, 아질산염과 같은 개별 염증표지자 및 SBI 진단 여부를 확인했다. 또, 응급실 방문 시점부터 28일 이내 사망, 응급실 퇴원, 중환자실 입원, 혈액배양 및 입원 결과를 의무기록을 통해 확인하고 평가하고, 필요하면 전화 인터뷰를 시행했다.

3. SBI 정의

1) 세균혈증: 혈액배양에서 단일 세균 확인
2) 급성 신우신염: 두 개의 연속적인 소변 검체(무균채뇨백)에서 단일 세균 105 colony-forming unit/mL 이상 확인
3) 엽폐렴: 영상의학과 의사 판독에 따른 단순흉부방사선사진에서 경화(consolidation)
4) 세균수막염: 뇌척수액 배양에서 세균 확인
5) 골수염 및 화농관절염: 자기공명영상에서 뼈 침범 증거 또는 혈액, 뼈, 활액 배양에서 세균 확인
6) 오염: 배양에서 coagulase-negative staphylococci, viridans group streptococci (감염심내막염 제외), Micrococcus spp., Corynebacterium spp., Neisseria meningitidis 또는 Neisseria gonorrhoeaeNeisseria spp.가 확인되면 오염으로 정의했다. 또한, 의무기록에 오염으로 기재됐거나 배양 양성에도 불구하고 항생제를 투여하지 않으면, 오염으로 간주했다[5,6].

4. The Laboratory-score

Laboratory-score는 C-반응단백질 4 mg/dL 미만, 4 mg/dL 이상 10 mg/dL 미만, 10 mg/dL 이상에 각각 0, 2, 4점을, 프로칼시토닌 0.5 μg/L 미만, 0.5 μg/L 이상, 2.0 μg/L 이상에 대해 각각 0, 2, 4점을, urine dip stick 양성에 1점을 각각 배점하고, 이를 합산하여(총점 0-9점) SBI의 발생을 예측한다. 본 연구에서는 농뇨(≥ 5 cells/high-power field) 또는 아질산염 양성을 1점을 배점하여 원전의 urine dip stick 기준을 대신했다(Appendix 1).

5. 염증표지자 결합모형

염증표지자 결합모형은 SBI를 예측하기 위해 본 연구에서 제시하는 모형이다. 연구대상자의 개별 염증표지자 중 적혈구침강속도, C-반응단백질, 프로칼시토닌을 포함하여 다중회귀분석을 시행했으며, 이를 통해 SBI 예측성적을 계산했다.

6. 통계적 방법

연속형 변수는 평균 및 표준편차 또는 중앙값 및 사분위수 범위로, 범주형 변수는 수와 백분율로 각각 표시했다. SBI군 및 비 SBI군 간 차이를 분석하기 위해 연속형 변수는 Student t-test 또는 Mann-Whitney U test를, 범주형 변수는 chi-square test를 각각 사용했다. 각 변수의 SBI 예측성적을 평가하기 위해 receiver-operating characteristic (ROC) 곡선을 그리고 곡선 아래 면적(area under the curve, AUC)을 구했으며 Youden index를 이용하여 적절한 절단값을 정하고, 민감도 및 특이도를 분석했다. 통계적 분석에는 SPSS ver. 16.0 (SPSS Inc., Chicago, IL)과 MedCalc (MedCalc Software, Mariakerke, Belgium)를 사용했다. P가 0.05 미만인 경우를 통계적 유의성이 있는 것으로 정의했다.

결과

1. 연구대상자의 특성

연구기간에 방문한 열 환자 7,968명에서 제외기준을 적용한 338명 중 응급실에서 검사를 시행한 203명을 연구대상자로 정의하여 분석했고, 이 중 SBI 환자는 58명(28.6%)이었다. 나이, 성별, 체온, 열 지속시간은 SBI군과 비 SBI군에서 유의한 차이가 없었다(Table 1). 모든 개별 염증표지자 값이 SBI군에서 유의하게 높았고, 이 중 프로칼시토닌은 SBI 및 비 SBI군에서 각각 0.5 μg/L (사분위수 범위, 0.3-0.9 μg/L)와 0.2 μg/L (사분위수 범위, 0.1-0.3 μg/L)였다(P < 0.001) (Table 1).

2. 임상결과와 SBI 발생 원인

입원율은 SBI군에서 93.1%로 비-SBI군보다 높았고(P < 0.001), 입원 기간의 중앙값도 더 길었다(7일[사분위수 범위, 6-8일] vs. 4일[사분위수 범위, 3-6일]; P = 0.035) (Table 2). 7명(3.4%)이 중환자실에 입원했고, 28일 이내 사망 환자는 없었다. SBI 중 급성 신우신염이 36명(62.0%)으로 가장 흔했고, 엽성 폐렴 12명(20.8%), 균혈증 6명(10.3%)이 그 뒤를 이었다(Table 3).

3. SBI 예측성적

백혈구수, 적혈구침강속도, C-반응단백질, 프로칼시토닌의 ROC 곡선을 그리고 AUC를 계산했다(Fig.1, Table 4). 프로칼시토닌은 절단값 0.303 μg/L에서 민감도 및 특이도가 각각 75.9% (95% 신뢰구간, 62.8%-86.1%) 및 74.5% (95% 신뢰구간, 66.6%-81.4%), AUC는 0.77 (95% 신뢰구간, 0.71-0.84; P < 0.001)로 개별 염증표지자들 중 가장 높은 AUC를 보였다.
Laboratory-score의 민감도 및 특이도는 각각 86.2% (95% 신뢰구간, 74.6%-93.9%) 및 52.4% (95% 신뢰구간, 44.0%-60.8%)였고, AUC는 0.76 (95% 신뢰구간, 0.69-0.81)으로 유의하게 SBI 발생을 예측했다(P < 0.001) (Table 5). 염증표지자 결합모형의 민감도와 특이도는 75.9% (95% 신뢰구간, 68.1%-82.6%) 및 72.4% (95% 신뢰구간, 59.1%-83.3%), AUC 0.80 (95% 신뢰구간, 0.74-0.86)으로 프로칼시토닌 및 Laboratory-score의 해당 값보다 높았다(P < 0.001) (Table 5, Fig. 2).

고찰

본 연구는 36개월 이하 국소증상 없는 열 환자의 SBI 예측에 있어서, 새로 개발한 염증표지자 결합모형이 개별 염증표지자인 프로칼시토닌 및 기존 점수체계인 Laboratory-score보다 우수하다는 점을 보여준다. 프로칼시토닌은 국소증상 없는 열 환자의 SBI 예측에 유용한 염증표지자임을 확인했다. 또한, Laboratory-score는 SBI 예측에 유용성을 보였으나, 염증표지자 결합모형이 더 우수한 예측성적을 보였다.
개별 염증표지자는 체온, 심장박동수, 호흡수, 혈압 등 다른 임상 변수보다 객관적이다. 혈청 염증표지자 농도를 일정 간격으로 연속 측정하면 시간에 따른 농도 변화를 대략 파악할 수 있고, 이 측정법이 단일측정 값보다 예측성적 면에서 우수하다고 알려졌다[7]. 그러나, 응급실에서 연속 측정이 현실적으로 어려우므로 절대값에 초점을 맞추면, 초기 프로칼시토닌은 국소증상 없는 열 환자에서 SBI 발생에 양호한 예측성적을 보이고 점수체계보다 간단하다. 프로칼시토닌은 바이러스 감염보다 SBI에 특이적으로 높은 농도를 보이고, 연속측정은 호흡기 감염 및 열성 질환 환자의 항생제요법 기간을 줄이는 것으로 나타났다[8]. 한편, 감염병 의심 환자에서 체온 상승[9-11], 말초혈액 백혈구 수 증가[12], C-반응단백질 농도 상승은[13] 프로칼시토닌보다 SBI 예측성적이 열등하다고 보고됐으며, 이는 본 연구 결과와 일치한다.
Nijman 등[14]은 국소증상 없는 열 소아환자에서 Laboratory-score의 SBI에 대한 민감도 및 특이도를 각각 94% 및 81%라고 보고했다[15]. 본 연구에서 Laboratory-score의 AUC는 0.76 (95% 신뢰구간, 0.69-0.81)으로 Nijman 등[14]이 보고한 0.79 (95% 신뢰구간, 0.72-0.87)보다 낮아 다른 임상환경에서 추가 검증할 필요가 있다고 생각한다.
본 저자는 민감도가 높게 나온 C-반응단백질, 특이도가 높게 나온 적혈구침강속도, 민감도 및 특이도가 모두 높게 나온 프로칼시토닌을 조합하여 염증표지자 결합모형을 제시했으며, SBI 예측성적을 비교 분석했다. 염증표지자 결합모형의 AUC는 0.80 (95% 신뢰구간, 0.74-0.87)으로 Laboratory-score보다 우수했으며, 이는 염증표지자 3종을 조합함으로써 개별 염증표지자가 갖는 민감도 및 특이도 면의 한계점을 보완한 것에 기인한 것으로 추정한다.
본 연구의 제한점은 다음과 같다. 첫째, 단일 기관 연구로 연구대상자 수(203명)가 상대적으로 적어, 연구결과를 일반화하기 어려울 수 있다. 다만, 다른 연구도 대상자가 300명 이하였다[3,16,17]. 향후 연구결과의 일반화를 위해 연구대상자 범위의 확대가 필요하다. 둘째, 후향적 의무기록 연구로, 기록 정확도에 따라 연구결과가 영향을 받을 수 있다. 그런데도, 염증표지자 결합모형은 개별 염증표지자 및 Laboratory-score보다 SBI 발생을 더 정확하게 예측했다. 따라서, 본 연구를 기반으로 진료 과정에 이 점수체계를 적용한 전향적 연구가 필요하다.
요약하면, 본 연구에서 36개월 이하 환자를 대상으로 SBI에 대한 예측성적을 비교한 결과, 염증표지자 결합모형이 개별 염증표지자 및 Laboratory-score보다 우수했다.
