Zusammenfassung

Das Cannabis-als-Medizin-Gesetz regelt seit 2017 die Verordnung von Cannabinoiden in Deutschland. In den meisten Studien wurde CBD (Cannabidiol) und / oder THC (Tetrahydrocannabinol) in ihrer Wirkung auf Schmerzen untersucht. Während die präklinische Studienlage antinozizeptive Resultate nachweisen konnte, ist die klinische Studienlage sehr heterogen und lässt keinen sicheren Schluss zu. Die Number Needed to Treat von 24 und die Number Needed to Harm von 6 legt nahe, dass die Risiken einen möglichen therapeutischen Nutzen übersteigen. Die Datenlage ist für Schlafstörungen oder Angststörungen bei Schmerzpatient:innen auch nicht klarer. Weiterhin ist die Studienlage für eine mögliche Reduktion der verschriebenen Opioiddosen durch die Applikation von CBM widersprüchlich. Ob eine kleine Gruppe von chronischen Schmerzpatient:innen von CBM profitieren könnte, ist bisher unklar, eine Leitlinienempfehlung ist notwendig, die sowohl die Indikationen für Beginn und Ende der Therapie als auch die abzuwägenden Risiken dem Behandlenden empfiehlt.

Abstract

The role of cannabis in the treatment of chronic pain
The German »Cannabis as Medicine« Act has regulated the prescription of cannabinoids for medical conditions in Germany since 2017. Most studies have examined CBD (cannabidiol) and / or THC (tetrahydrocannabinol) for their effects on acute and chronic pain. While the preclinical studies demonstrated antinociceptive results, the clinical studies are very heterogeneous and do not allow for clear conclusions. The Number Needed to Treat of 24 and the Number Needed to Harm of 6 suggest that the risks outweigh any potential therapeutic benefits. What the data shows is no clearer for sleep disorders or anxiety disorders in pain patients. Furthermore, on balance the studies are contradictory as regards a possible reduction in opioids through the application of CBM. It is still unclear whether a small group of chronic pain patients could benefit from CBM; a recommendation for a guideline is necessary that lays out the indications for the start and end of therapy as well as the risks to be weighed up to the practitioner.
