비자살적 자해(Non-suicidal Self-Injury: NSSI)는 죽음의 의도 없이 고의적으로 자신의 신체조직을 손상시키는 행위이다(Nock, 2009). 최근 청소년들의 자해 사례들이 증가되고 있는 추세에 있는 반면 국내에서는 청소년 자해와 관련된 연구가 부족한 가운데 있다. 본 연구는 자해와 관련된 국외 및 국내의 선행연구들을 개관하고 비자살적 자해에 영향을 미치는 위험요인과 보호요인을 살펴보고자 하였다. 우선 비자살적 자해를 설명하는 대표적인 이론 중에서 4가지 기능 모델, 통합적 이론 모델, 경험회피 모델을 통해, 왜 고통에도 불구하고 자해를 반복하는지 이론적인 측면에서 알아보고자 하였다. 선행연구들을 통해 자해의 위험요인으로는 아동기의 양육환경, 정서조절의 어려움, 부정적인 자기상(self-image), 정신의학적 상태, 심리사회적 요인을 중심으로 정리하였고, 자해의 보호요인으로는 상처에 대한 인식, 삶의 목표, 자기(self)의 회복, 사회적 지지를 중심으로 정리하였다. 나아가 자해의 예방과 상담을 위한 보다 실제적인 방향을 제시하고, 추후 연구를 위한 과제 등을 논의하였다.

Non-suicidal self-injury(NSSI) is an act of causing damage to one's own body part intentionally without the idea of death(Nock, 2009). There is a recent rising trend of self-injury cases among adolescents, but Korea is in shortage of research on the self-injury of adolescents. This study thus set out to review previous studies on self-injury home and abroad and examine risk and protection factors to influence the NSS. First of all, the study investigated the reasons why they would repeat self-injury despite the pain in theoretical aspects with the four-function model, integrated theory model, and experience avoidance model, which are representative theories to explain NSSI. The findings were arranged around the risk factors of self-injury including the upbringing environment during childhood, difficulties with emotional regulation, negative self-image, psychiatric state, and psychosocial factors and also the protection factors of self-injury including the perceptions of wounds, life goals, recovery of self, and social support. Furthermore, it proposed more practical directions for the prevention and counseling of self-injury and discussed tasks for follow-up study.
