초록

본 연구는 문헌연구를 통해 노인 장기요양서비스의 질에 대한 측정상의 쟁점을 고찰하고, 이를 토대로 현재 우리나라 노인 장기요양서비스의 질 평가에 대한 시사점을 도출함을 목적으로 하였다. 먼저 노인 장기요양서비스의 질에 대한 개념적 접근 및 사정에 대한 분석적 틀에 대해서 고찰한 다음, 장기요양서비스의 질을 사정함에 있어 시설보호와 재가보호에서 차별적으로 고려해야 할 사항을 살펴보았다. 다음으로, 서비스의 질에 대한 측정상의 쟁점에서는 과정지표 대비 성과지표, 성과지표의 쟁점(이용자 만족도 및 삶의 질), 면접대상자 선정(이용노인 대비 대리인), 기타 통계자료 산출에 관련한 내용(발생율 산출을 위한 통계산출기간 설정 및 자료의 절단)에 대하여 살펴보았다. 장기요양서비스의 질에 대한 개념 및 측정상의 쟁점을 중심으로 우리나라 장기요양서비스의 질에 대한 평가체계의 문제점에 대해서 살펴보고, 평가에 대한 시사점을 다음과 같이 도출하였다: 서비스의 질에 대한 개념 명료화, 시설보호와 재가보호의 서비스의 질에 대한 차별적인 지표 구성, 구조･과정･성과지표의 개선, 이용노인 중심의 평가수행, 통계자료 산출의 개선방향. 결론에서는 장기요양서비스의 질에 대한 경험적 연구 및 종단적 자료 구축의 필요성에 대해서 논하였다.
