Influence of low tital volume ventilation on curative effect and prognosis of patients with high risk ALI / ARDSChinese Full Text

Abstract: Aim: To explore the influence of low tital volume ventilation on curative effect and prognosis of patients with high risk acute lung injury( ALI) / acute respiratory distress syndrome( ARDS). Methods: From August,2012 to May,2013 in the Fifth Affiliated Hospital of Zhengzhou University,according to the inclusion standard,we selected 68 patients with high risk ALI / ARDS in Respiratory Intensive Care Unit( RICU). All the patients were allocated into two groups: 36 patients( low tidal volume group) were given low tidal volume ventilation( VT 6 8 mL / kg),while 32 patients( conventional tidal volume group) were given conventional tidal volume ventilation( VT 10 12 mL/kg). The indicators such as blood gas analysis results,the expression levels of inflammatory mediators( TNF-α,IL-6) in the serum and bronchoalveolar lavage fluid were collected respectively at 0 h,48 h,96 h after mechanical ventilation; the incidence of ALI / ARDS,duration of mechanical ventilation,RICU stay,hospital stay and total mortality in the two groups were compared. Results: Compared with initiation of mechanical ventilation in both groups,the levels of TNF-α and IL-6 in serum decreased gradually,and the levels of TNF-α and IL-6 in bronchoalveolar lavage fluid declined significantly at 96 h after mechanical ventilation; compared with the conventional tidal volume group,the levels of inflammatory mediators in the low tidal volume group declined more significantly( P < 0. 05). Compared with the conventional tidal volume group,the incidence of ALI / ARDS,duration of mechanical ventilation,RICU stay,hospital stay and total mortality of the low tidal volume group were all improved( P < 0. 05). Conclusion: For the patients with high risk ALI / ARDS,ALI / ARDS may be induced by mechanical ventilation. But for the patients who must be treated by mechanical ventilation,low tidal volume ventilation could bring about better effect and prognosis.
