Diagnostic value of high-frequency three-dimensional ultrasound to the rupture of deltoid ligamentChinese Full Text

Abstract: Objective To explore the value of high-frequency three-dimensional ultrasound to the diagnosis of deltoid ligament rupture. Methods High-frequency three-dimensional ultrasound and MRI examination were performed respectively in 68 patients with suspected deltoid ligament rupture.The surgical probing results were used to determine the deltoid ligament rupture,and the coincidence rates of the two methods were compared.Results Forty patients were diagnosed deltoid ligament rupture by high-frequency three-dimensional ultrasound,mainly demonstrating totally discontinuous medial ligaments of ankle joint,non-echo areas in some regions,distinctly increased intervals between nonecho areas in varus test,and increased interval from low-echo area to non-echo area in probing in valgus/varus ligament test.Forty-one patients were diagnosed deltoid ligament rupture by MRI,which presented discontinuous ligament,ligamentous laxity and wave-like change,and abnormally intense signal in ligament.Deltoid ligament probing operation should be performed if deltoid ligament rupture was indicated by either of the detection methods.Forty-two cases were confirmed deltoid ligament rupture and performed repair operation,and the left 26 cases received conservative treatment.High-frequency three-dimensional ultrasound showed positive in 39 cases and false positive in 1case,with the coincidence rate of 92.85%(39/42).MRI showed positive in 40 cases and false positive in 1case,with the coincidence rate of95.23%(40/42).Conclusion High-frequency three-dimensional ultrasound has similar coincidence rate to MRI,with the advantages of convenience and good reproducibility.
