Abstract: Objective To explore the clinical value of nasotracheal intubation mechanical ventilation guided by bronchofiberscope for the treatment of chronic obstructive pulmonary disease( COPD) complicated with respiratory failure. Methods Eighty-six patients with severe COPD complicated with respiratory failure and reached the indications of invasive ventilation treatment were randomly divided into two groups( n = 43 each) : experimental group and control group. The nasotracheal intubation mechanical ventilation guided by bronchofiberscope was used in experimental group,and the orotracheal intubation mechanical ventilation was used in control group. The one-time cannula success rate,patients,tolerability,incidence of adverse reactions,needed time to reach weaning criteria and other relevant indications in the two strategies were compared. Results Compared with the control group,the one-time cannula success rate significantly increased( 93. 0% vs 74. 4%,P < 0. 05); the time to operate significantly decreased[( 3. 06 ± 0. 87) min vs( 4. 22 ± 0. 91) min,P < 0. 01]; patients’ tolerance rate significantly increased( 76. 7% vs 51. 2%,P < 0. 05); the incidence of adverse reactions significantly lowered( 32. 6 % vs58. 1%,P < 0. 05); the needed times to reach normal levels of blood gas analysis indications( SaO2,PaCO2) for the first time and to reach weaning criteria significantly decreased all[( 25. 38 ± 10. 15) h vs( 32. 12 ± 11. 62) h,P <0. 01;( 3. 22 ± 1. 46) d vs( 4. 37 ± 1. 61) d,P < 0. 01] in the experimental group. Conclusions Compared with orotracheal intubation mechanical ventilation for the treatment of severe COPD complicated with respiratory failure,the strategy using nasotracheal intubation mechanical ventilation guided by bronchofiberscope has a good tolerability for patients and a satisfactory therapeutic effect in clinic.
