　腹腔鏡下膵臓切除術におけるLigaSure^TMの効用について検討した．7頭の成犬に膵体尾部切除術を行い，1週間後に再開腹してLigaSure^TMで切離した膵臓断端からの膵液漏の有無を観察したところ，7例中3例の切離断端に嚢胞形成を認めたため，膵液漏の回避にはlaparosonic coagulating shears（LCS）と同様に主膵管の結紮は省略できないと思われた．膵臓へのアプローチに必要な胃横行結腸間膜切離についてLiga Sure^TM使用群と非使用群で比較したが，所要時間は使用群が10.4分，非使用群が21分と約1／2に短縮された．使用群は途中出血もなく，両彎曲部間の直線的な切離が可能であるなど，間膜処理での有用性が際立っていた．また，後腹膜アプローチによる膵体尾部切除術を紹介した．

We examined the following factors concerning the advantage of LigaSure^TM (LVSS), prevention of pancreatic juice leakage from the stump of pancreas in laparoscopic pancreatectomy and shortening of time reguired to separate the greater omentum. Distal pancreatectomy using LVSS was performed on seven dogs. One week later, the stumps of the pancreas were observed. In 3 cases, cyst formation was observed, which indicates that pancreatic juice leakage occurred.
