Safe placement of ports, effective adhesiolysis and adequate bowel resection with or without lymphadenectomy are mandatory in laparoscopic colorectal surgery, especially for patients with a past history of abdominal operation. The first port should be inserted by open laparoscopy technique in one of the port-sites marked for the scheduled laparoscopic colorectal resection. It should also be freed from adhesions around the previous surgical scars. All the other ports should be placed in position followed by gentle lysis of dense adhesion without any injury of the intestines.
