要約　目的：裂孔原性網膜剝離に対して行った25ゲージ硝子体手術の結果の報告。対象と方法：過去22か月間に初回手術として25ゲージ硝子体手術を行った裂孔原性網膜剝離77例77眼を対象とした。男性44例，女性33例で，平均年齢は58歳である。手術は6名の術者で行い，6か月間以上の経過を追った。結果：初回手術で73眼（95％），最終的に77眼すべてで網膜復位が得られた。小数視力は術前の0.61が術後0.99に改善した（p＜0.05）。術後に感染性眼内炎が1眼（1.3％）に生じたが，他に重篤な合併症はなかった。結論：裂孔原性網膜剝離に対して当科で行った25ゲージ硝子体手術で，全例に網膜復位が得られた。

Abstract.　Purpose：To report the outcome of 25-gauge vitrectomy for rhegmatogenous retinal detachment. Cases and Method：This retrospective study was made on a consecutive series of 77 eyes of 77 cases who received 25-gauge vitrectomy as the primary procedure for rhegmatogenous retinal detachment during the past 22 months. The series comprised 44 males and 33 females. The age averaged 58 years. Surgery was performed by one of 6 surgeons. Cases were followed up for 6 months or longer. Results：The retina became reattached in 73 eyes（95％）after the primary procedure and in 77 eyes（100％）after multiple procedures. Averaged value of decimal visual acuity improved from 0.61 before surgery to 0.99 after surgery（p＜0.05）. Infectious endophthalmitis developed in one eye（1.3％）. Otherwise,there was no serious complication in the series. Conclusion：Retinal reattachment was obtained in all eyes after single or multiple sessions of 25-gauge vitrectomy.
