要旨　注腸X線検査は，容易に腸管の走行や病変の位置が把握できるとともに，3次元的な構造が理解しやすく直腸肛門部の検査には有用と考えられる．しかし，意識して検査を行わないと病変を見逃すことが多く，よい画像を撮影することも難しい．一方，CTやMRIにおける最近の進歩は著しく，直腸肛門部の診断には欠かせなくなってきている．直腸肛門部周囲の筋群の解剖は複雑であり，病変との位置関係を把握するためには，組織コントラストに優れたMRIを利用するべきである．しかし，MRIでは撮像される範囲が狭くなることが多く，より広い範囲の診断が必要な場合には，CTを用いるか，CTとMRIを組み合わせる必要がある．

　Barium enema brings us a lot of information, especially concerning the anatomy of the colon and rectum, and situation, size and morphology of anorectal lesions.

　Either CT or MRI is useful for evaluation of anorectal lesions. MRI is superior to CT for depicting the lesions, and especially for understanding the anatomical relationship between surrounding muscles and anal fistula related to Crohn's disease. However,MRI has a limitation in scan range. Therefore,CT examination is recommended to evaluate the presence of metastatic lesions or to detect active inflammation of the alimentary tract. The anatomy of the anal canal and supporting muscles is explained in Fig. 6.
