Zusammenfassung
Die Behandlung von Augenerkrankungen mittels Akupunktur kann nach Regeln der traditionellen chinesischen Medizin oder aber relativ unabhängig von der individuellen Situation nach feststehenden Therapiemustern erfolgen. In Europa erhielt letztere Akupunkturform durch Entwicklungen zweier dänischer Akupunkteure – Freddy Dahlgren und John Boel – seit 1980 entscheidende neue Impulse. Das Prinzip dieser Augenakupunktur beruht auf einer Kombination der Mikroakupunktursysteme von ECIWO, NPSO, SU JOK, YNSA und Aurikulotherapie.
In diesem Artikel wird exemplarisch das Vorgehen beider Akupunkturformen dargestellt.
Welche Therapieform, welche Akupunkturpunkte und Punktekombinationen sowie Stimulationsmethoden tatsächlich die optimale Wirkung bei Augenerkrankungen haben, benötigt weitere wissenschaftliche Forschung.
Abstract
The treatment of eye diseases with acupuncture may follow either the classical tradition of Chinese medicine or more modern regimens which do not address the individual over-all situation but use fixed acupoint schemes for specific eye disorders. This modern form of treatment has been brought forward since 1980 by two Danish acupuncturists, Freddy Dahlgren and John Boel. Their treatment options are based on a combination of microsystem acupuncture: ECIWO, NPSO, SU JOK, YNSA and auriculotherapy.
This article focuses on both regimens of treatment.
More scientific research is needed to evaluate which form of treatment, which acupoints and acupoint combinations, and which forms of stimulation show the best results in treating auricular disorders with acupuncture.

Female patient with age related macular degeneration (AMD)
2012, Deutsche Zeitschrift fur AkupunkturAltersabhängige Makuladegeneration (AMD) stellt in den Industrienationen derzeit die häufigste Form von massiven Sehbeeinträchtigungen im Alter dar. Konventionelle Therapien bieten betroffenen Patienten oft keine ausreichende Option, wodurch sich die Frage ergibt, ob Akupunktur bei AMD eine sinnvolle Ergänzung sein kann. In Internetforen wird Akupunktur oft erwähnt, wobei aber auch angemerkt ist, dass wissenschaftliche Nachweise über die Wirksamkeit bis dato noch nicht erbracht werden konnten. Dieses Expertenforum soll sechs Jahre nach dem ersten Bericht über eine Patientin mit genetisch bedingter AMD ein Update der entsprechenden Literatur geben, über das Befinden der damals vorgestellten Patientin berichten und anhand eines neuen Fallberichtes die Therapieempfehlungen zweier Experten aufzeigen.
Age related macular degeneration (AMD) is the most frequent reason for loss of visual function in industrial nations. Conventional therapies provide no optimal strategies, so it is useful to know if acupuncture is a useful option as an additive treatment. In Internet forums acupuncture often is mentioned but there still is a lack of scientific evidence. In 2006 the Clinician's corner in DZA discussed a female patient with AMD for the first time, now, six years later, there should be an update.
Acupuncture may improve vision in patients with age-related macular degeneration (AMD): An observational study
2009, Revista Internacional de AcupunturaLa degeneración macular asociada a la edad (DMAE) es una enfermedad que va destruyendo de manera progresiva la visión central en personas de edades avanzadas. Las posibilidades de tratamiento son limitadas y a menudo la pérdida de agudeza visual es inevitable. En el presente estudio se han investigado los efectos de un tratamiento concreto de acupuntura sobre la agudeza visual de pacientes con DMAE.
Se reunieron 328 pacientes diagnosticados clínicamente de DMAE y se les trató con acupuntura durante un período de 2 semanas (de lunes a viernes), 2 veces al día, dejando un espacio mínimo de 60 min entre las sesiones. Se midió la agudeza visual antes del tratamiento, y tras una y 2 semanas de tratamiento utilizando una prueba de agudeza visual estándar a una distancia larga (3 m) y otra corta (40 cm).
La agudeza visual media mejoró después de la primera semana de tratamiento a las 2 distancias medidas (p < 0,001). Después de la segunda semana de tratamiento se observó una mejoría adicional (p < 0,001). Durante las 2 semanas de tratamiento, la visión mejoró a la distancia de 3 m en 145 pacientes (44,2%) y a 40 cm en 290 pacientes (88,4%).
La acupuntura podría representar una opción de tratamiento adecuada para pacientes con DMAE. Se requieren estudios aleatorizados con doble enmascaramiento y controlados con placebo para llegar a la demostración definitiva de la eficacia de la acupuntura para el tratamiento de la DMAE.
Age-related macular degeneration (AMD) is a disease associated with aging that gradually destroys sharp, central vision. Treatment options for AMD are limited and often vision loss cannot be prevented. The present study was performed to evaluate the effects of acupuncture treatment on vision in AMD patients.
A total of 328 patients with a clinical diagnosis of AMD were treated over a period of 2 weeks (Monday to Friday) with acupuncture given two times per day with a minimum time of 60 min between treatments. Vision was evaluated at baseline, after one and after two weeks of treatment by a distant (3 m) and at near (40 cm) standard reading test.
Median visual acuity improved during the first treatment week at both distances (p < 0.001). Further improvement of visual acuity was observed during the second week (p < 0.001). From baseline examination to the final examination, vision at 3 m distance improved in 145 (44.2 %) patients and vision at 40 cm distance improved in 290 (88.4 %) patients.
We conclude that acupuncture may be a useful treatment option in patients with AMD. Double-blinded, randomized sham-controlled studies will be necessary for final proof of action.
Influence of acupuncture on intraocular pressure and compliance of patients with ocular hypertension or primary wide-angle glaucoma: First results of a controlled prospective follow-up study
2008, Revista Internacional de AcupunturaEl glaucoma primario de ángulo abierto, la forma más frecuente de glaucoma, es una enfermedad multifactorial del nervio óptico. La pérdida de células ganglionares de la retina ocasiona en última instancia la atrofia del nervio óptico. A este respecto, la presión intraocular (PIO) se considera el parámetro de riesgo más importante y más susceptible de tratamiento. El objetivo de este estudio es analizar la influencia de la acupuntura sobre la presión ocular en pacientes con glaucoma primario de ángulo abierto e hipertensión ocular y comprobar su adhesión al tratamiento.
Se formaron 3 grupos de estudio: pacientes con glaucoma primario de ángulo abierto (n = 21), y además con hipertensión ocular (n = 13) en comparación con un grupo de referencia sin enfermedades oculares (n = 16). Antes y después del tratamiento de acupuntura se realizó una valoración oftalmológica de la agudeza visual según DIN 58220 y un examen estático del campo visual asistido por ordenador. También se realizó antes del inicio del tratamiento una paquimetría. La PIO se midió antes de cada sesión de acupuntura, nada más concluirla, así como 30 min después. El cumplimiento fue valorado mediante un cuestionario estandarizado de paciente.
La acupuntura redujo de forma significativa alargo plazo la PIO en pacientes con glaucoma primario de ángulo abierto e hipertensión ocular, en comparación con un grupo de control de individuos sin enfermedad ocular. En el grupo de pacientes con glaucoma, el cumplimiento terapéutico aumentó gracias a la acupuntura y la satisfacción vital general se vio positivamente influenciada.
La acupuntura es adecuada como método terapéutico coadyuvante para el tratamiento de pacientes con glaucoma primario de ángulo abierto. En caso de hipertensión ocular, la acupuntura puede emplearse para disminuir la PIO bajo un control del campo visual y una evaluación objetiva de los cambios micromorfológicos en la papila, sobre todo en pacientes que tienen contraindicados los fármacos antiglaucomatosos (bloqueantes β), o en caso de alergia a los conservantes o aversión a los tratamientos farmacológicos. De esta forma se puede reducir la tasa de conversión a un glaucoma manifiesto.
Primary wide-angle glaucoma (the most common form of glaucoma) is a multi-factorial disease of the optic nerve marked by the preceding loss of retinal ganglion cells, eventually leading to the atrophy of the optic nerve. Intraocular pressure (IOP) makes up the most important and most treatable risk factor.
The study examined the eff ect of acupuncture therapy on eye pressure and the compliance in patients suff eringfrom primary wide-angle glaucoma (n = 21) and ocular hypertension (n = 13) against a group of test subjects not suff eringfrom any eye complaint (n = 16). Before commencing therapy, corneal thickness was determined by 4 Optics-Pachymeter. Before and after acupuncture treatment, an ophthalmologic determination of visual acuity according to DIN 58220 and a computer-aided visual field analysis were done. The IOP wasmeasured before and immediately after acupuncture as well as30 minutes after therapy. Changes in compliance were documented with a standardized questionnaire completed by eachpatient having received this treatment.
It was shown that for patients with primary wide angle glaucoma and ocular hypertension, acupuncture is able to significantly lower intraocular pressure. According to the questionnaire analysis, a subjective improvement of quality of life and better compliance of using anti-glaucomatous medication was achieved.
Acupuncture is an appropriate method to treat patients with primary wide-angle glaucoma in addition tolowering eye pressure by anti-glaucomatous pharmacologicaltherapy. In patients suff ering from ocular hypertension, acupuncture can significantly lower intraocular pressure and therefore reduce the rate of conversion into a manifest glaucoma. Therefore this treatment can be suggested for ocular hypertension patients with contraindications against localanti-glaucomatous medicine (e. g. beta-blocker). However, it is absolutely necessary in this treatment that patients are continuously monitored regarding visual field, retinal nerve fibre thickness and micromorphological alterations of the disc in order to detect tissue damage of the optic nerve and functional loss caused by the onset of glaucoma.

Die altersbedingte Makula Degeneration (AMD) ist eine Krankheit, die im fortgeschrittenen Alter allmählich das zentrale Scharfsehen zerstört. Die Behandlungsoptionen sind limitiert, der Verlust der Sehkraft kann oft nicht verhindert werden. In der vorliegenden Studie wurden die Effekte einer Akupunktur-Behandlung auf die Sehkraft von AMD Patienten untersucht.
Insgesamt wurden 328 Patienten mit einer klinischen Diagnose von AMD über einen Zeitraum von zwei Wochen (jeweils Montag bis Freitag) zwei Mal täglich mit einem Mindestabstand von 60 Minuten zwischen den Behandlungen akupunktiert. Die Sehkraft wurde vor der Behandlung, nach einer Woche und nach zwei Wochen mittels eines Standard Lese-Tests auf große (3 m) und geringe (40 cm) Entfernung gemessen.
Die mittlere Sehkraft verbesserte sich nach der ersten Behandlungswoche bei beiden Entfernungen (p < 0.001). Eine weitere Verbesserung der Sehkraft wurde nach der zweiten Behandlungswoche beobachtet (p < 0.001). Während der zwei Behandlungswochen verbesserte sich die Sehkraft auf 3 m Entfernung bei 145 (44,2%) Patienten und auf 40 cm bei 290 (88,4%) Patienten.
Akupunktur könnte eine sinnvolle Behandlungsoption für Patienten mit AMD sein. Doppel-blinde, randomisierte placebo-kontrollierte Studien sind nötig, um einen definitiven Beweis für die Wirksamkeit von Akupunktur zur Behandlung der AMD zu erbringen.
Influence of acupuncture on intraocular pressure and compliance of patients with ocular hypertension or primary wide-angle glaucoma: First results of a controlled prospective follow-up study
2008, Deutsche Zeitschrift fur AkupunkturDas primäre Offenwinkelglaukom, die häufigste Glaukom-Form, ist eine multifaktorielle Erkrankung des Sehnervs. Der Verlust retinaler Ganglienzellen führt letztendlich zur Atrophie des Nervus opticus. Der intraokulare Druck (IOD) gilt dabei als der wichtigste und am besten zu beeinflussende Risikoparameter. Es sollte der Einfluss der Akupunktur auf den Augendruck bei Patienten mit primärem Offenwinkelglaukom und okulärer Hypertension sowie deren Compliance geprüft werden.
Es wurden drei Untersuchungsgruppen gebildet: Patienten mit primärem Offenwinkelglaukom (n = 21), mit okulärer Hypertension (n = 13) im Vergleich zu einer augengesunden Vergleichsgruppe (n = 16). Vor und nach der Akupunkturbehandlung erfolgten eine augenärztliche Bestimmung des Visus nach DIN 58220, eine statische computergestützte Gesichtsfelduntersuchung sowie vor Therapiebeginn eine Pachymetrie. Der IOD wurde vor, unmittelbar nach sowie 30 Minuten nach jeder Sitzung gemessen. Die Compliance wurde mittels standardisiertem Patientenfragebogen evaluiert.
Akupunktur senkte bei Patienten mit primärem Offenwinkelglaukom und okulärer Hypertension im Vergleich zur augengesunden Kontrollgruppe den intraokularen Druck längerfristig signifikant. In der Glaukom-Gruppe stieg durch die Akupunktur die Compliance, die allgemeine Lebenszufriedenheit wurde positiv beeinflusst.
Die Akupunktur ist geeignet, als adjuvante Therapiemethode bei der Behandlung von Patienten mit primärem Offenwinkelglaukom eingesetzt zu werden. Bei der okulären Hypertension kann die Akupunktur unter Kontrolle des Gesichtsfeldes und objektiver Beurteilung mikromorphologischer Papillenveränderungen zur Senkung des intraokularen Druckes angewendet werden, insbesondere bei Patienten mit Kontraindikationen gegen lokale Antiglaukomatosa (Betablocker), bei Konservierungsmittelallergien oder Abneigung gegenüber einer Pharmakotherapie. Hierdurch kann die Rate des Konvertierens in ein manifestes Glaukom gesenkt werden.
