Zusammenfassung

Eine ausreichende Gewebeperfusion ist nicht nur Voraussetzung für eine erfolgreiche Wundheilung, sondern auch für alle Arten der Rekonstruktion in der Plastischen Chirurgie. Daher besteht ein erhebliches Interesse am Einsatz des Behandlungsprinzips der therapeutischen Angiogenese mit dem Ziel, die Überlebensrate bei ischämischen Lappenplastiken und auch die Wundheilung zu verbessern. Unterschiedliche Wachstumsfaktoren wurden bereits in experimentellen Modellen von Lappenplastiken erprobt. Klinisch wurde die Angiogenesetherapie besonders detailliert bei Herz-Kreislauf-Erkrankungen untersucht. Mehrere klinische Studien am Patienten sind in diesem Bereich noch nicht abgeschlossen. Die Ergebnisse dieser Untersuchungen werden aber auch für mögliche klinische Studien am Patienten im Bereich der Plastischen Chirurgie von großem Nutzen sein. Zusätzlich verspricht die Anwendung der Therapiestrategie im Bereich der kontrollierten, lokalen Lymphangiogenese eine neue Behandlungsmöglichkeit für das Lymphödem.

Abstract

Sufficient tissue blood perfusion is critical for wound healing and for reconstructions in plastic surgery. There is considerable interest in the use of therapeutic angiogenesis to improve survival of ischemic flaps and to improve wound healing. Several growth factors have been tested in experimental flap models. Clinically, angiogenic therapy has been extensively studied in cardiovascular ischemic diseases and several clinical studies are ongoing in human patients. Results from these cardiovascular trials will benefit the planning of clinical trials in the field of plastic surgery. Furthermore, the development of strategies for local and controlled induction of lymphangiogenesis will hopefully improve the treatment of edema.
