Type 2 Diabetes remission by weight loss is associated with changes of T cells indicating re- programming of the adaptive immune system
Aims Type 2 diabetes (T2D) is related to chronic low-grade inflammation and innate immune system dysfunction. The underlying mechanisms of immune-metabolic programming in diabetic remission by weight loss are still unclear. We therefore performed fluorescence-activated cell sorting (FACS) of peripheral blood mononuclear cells (PBMC) and determined inflammatory bio-molecules in T2D patients with diabetes remission after weight loss by hypocaloric diet.
Methods 36 overweight /obese patients with insulin-naïve T2D were recruited and underwent a 3-month very-low-calorie diet of 600-800 kcal/day to achieve 15kg weight loss. Anthropometry and mixed meal tests were performed at baseline (V1), after 1 week (V2) and 3 months (V3) of intervention. PBMCs were analyzed by FACS for T-cell phenotypes (CD3, CD4, CD8, CCR7, CD45RA) and 72 cytokines/chemokines by OLINK.
Results The frequency of CD3+ T cells increased significantly at V2 (p<0.01) and V3 (p<0.01) while CD4+ T helper and CD8+ T cytotoxic didn’t differ. The naïve pool in both CD4+ and CD8+ populations increased significantly at V2 (p<0.001) followed by a prominent reduction at V3 (p<0.001). CD8+ EMRA decreased significantly at V2 and went back to baseline at V3 while CD4+ EM and EMRA pools remained unchanged. IL-7 and several other cytokines decreased at V2 but increased back to baseline at V3 with no significant difference from baseline [1] [2] [3] [4].
Conclusions The whole population of T cells increased after short- and long-term fasting with 15kg weight loss. CD8+ cytotoxic T cells were changed more profoundly compared to CD4+ T helper cells.
