Strategien zur Versorgung von RhD-negativen Patienten mit RhD-positiven Erythrozytenkonzentraten

Zusammenfassung

Die Versorgung von RhD-negativen Patienten stellt eine zunehmende Herausforderung für nahezu alle transfundierenden Einrichtungen in Deutschland dar, verschärft durch den bundesweiten Rückgang der Spendezahlen von ca. 10% gegenüber den Spendezahlen vor der Coronapandemie. In dem Praxistipp wird sowohl auf die Risikoevaluierung einer Immunisierung gegen das RhD-Merkmal eingegangen als auch auf die verschiedenen Versorgungsstrategien zum einen bei unbekannten Patienten in der Notfallversorgung, als auch bei RhD-negativen Patienten mit semielektivem oder chronischem Transfusionsbedarf. Ferner werden die internationalen Leitlinien in Bezug auf die Umstellungspraxis vorgestellt und verglichen.

Abstract

The supply of RhD negative patients represents an increasing challenge in almost all transfusion facilities in Germany, exacerbated by the nationwide decline in blood donations of approximately 10% compared to donation numbers before the corona pandemic. In this article we evaluate the risk of immunization against RhD and hemolytic disease of the newborn after supply with RhD positive red blood cell concentrates and the various strategies for unknown patients in emergency situations, as well as for RhD negative patients with semi-elective or chronic transfusion needs. Furthermore, the international guidelines are presented and compared with regard to the conversion practice.
