Ungewöhnlicher Verlauf einer Candidiasis des Zentralnervensystems

Zusammenfassung
Ein halbes Jahr nach einer Pyelonephritis, Adnexitis und Soorkolpitis trat bei einer 18jährigen Patientin eine Bewußtseinstrübung auf. Die neurologische Untersuchung ergab eine hirnorganische Wesensänderung, eine diskrete Anisokorie und einen fraglichen Meningismus. Die Computertomographie des Schädels ließ einen Hydrocephalus sowie Zeichen eines erhöhten Hirndrucks erkennen. Der Liquor enthielt 2336/3 Zellen; der Gesamteiweißgehalt war auf 7,0 g/l erhöht, die Lactatkonzentration auf 6,95 mmol/l. Die Glucosekonzentration betrug im Liquor 51 mg/dl bei einem Serumwert von 75 mg/dl. Unter dem Verdacht auf eine tuberkulöse Meningitis erfolgte zunächst eine tuberkulostatische Vierfachtherapie, die jedoch nicht zur Besserung führte. Erst 5 Wochen später konnten im Liquor mikroskopisch Pilzsporen und nicht-verzweigte Hyphen nachgewiesen werden. Der Titer im Candida-Hämagglutinationstest des Liquors betrug maximal 1 : 2048. Die Liquorkultur erbrachte Candida albicans. Der Verlauf war kompliziert durch Nebenwirkungen der Antimykotika (Amphotericin B zwischen 4,5 und 45 mg/d, Flucytosin, viermal 1,7 g/d) und rezidivierende Verklebungen im Ventrikelsystem, die wiederholte neurochirurgische Interventionen erforderten. Nach über siebenmonatiger stationärer Behandlung konnte jedoch eine völlige Heilung erreicht werden.
Abstract
Six months after an attack of pyelonephritis, adnexitis and candida colpitis an 18-year-old girl developed some clouding of consciousness. On neurological examination she showed organic behavioural changes, discrete anisocoria and possible meningism. Computed tomography revealed hydrocephalus and signs of increased cerebrospinal fluid (CSF) pressure. CSF contained 2336/3 cells, while total protein was raised to 7.0 g/l and lactate concentration to 6.85 mmol/l. Glucose concentration in CSF was 51 mg/dl and 75 mg/dl in serum. As tuberculous meningitis was suspected, treatment was started with four tuberculostatic drugs, but there was no improvement. Five weeks later microscopic CSF examination showed fungal spores and non-branching hyphae. The maximal candida haemagglutination titre in CSF was 1 : 2048. CSF culture grew Candida albicans. The further course was complicated by side effects to the antimycotic drugs (amphotericin B between 4.5 and 45 mg daily; flucytosine 1.7 g four times daily) and recurrent obstruction in the ventricular system requiring repeated neurosurgical interventions. However, full cure was achieved after seven months' hospital treatment.
