Zusammenfassung

Bei der Therapie der schweren diffusen Peritonitis muss nach wie vor mit einer Letalität von bis zu 50 % gerechnet werden. Besonders die postoperative Form der Peritonitis zeigt nach dem Übergang in ein Sepsissyndrom eine hohe Sterblichkeit. Ziel der chirurgischen Therapie ist eine definitive Herdsanierung der Ursache der Peritonitis. Im Anschluss daran erfolgt die mechanische Behandlung der peritonealen Blätter mit Entfernung der Fibrinbeläge und Ausspülung des peritonealen Exsudates. In Abhängigkeit von der Schwere der peritonealen Entzündungsreaktion und eines abdominellen Kompartmentsyndroms kann das Abdomen offen gelassen, programmiert lavagiert oder primär verschlossen werden. Durch wiederholte Spülungen wird versucht, Einfluss auf die Entzündungsreaktion des Peritoneums zu nehmen. Die systemische Sepsisreakton wird intensivmedizinisch mit Organersatz und Wiederherstellung der Homöostase behandelt.

Abstract

The treatment of severe diffuse peritonitis is still associated with a mortality of up to 50 %. Particularly the postoperative peritonitis shows high mortality rates due to septic organ failures. The aim of surgical treatment is the definitive source control followed by removal of fibrin and abdominal lavage of contaminants and infectious fluids. Dependent on the severity of the peritoneal reaction, further treatment consists of primary abdominal closure and relaparotomy on demand, programmed lavage or laparostomy respectively. Septic complications have to be treated by intensive care medicine.
