Zusammenfassung

Der Fallbericht beschreibt Diagnose und Therapie eines Hodgkin-Lymphoms in der Schwangerschaft. Bei einer 24-jährigen II. Gravida I. Para war in der 28 + 1 SSW ein Hodgkin-Lymphom (Stadium 2 A) diagnostiziert und histologisch gesichert worden. Nach Lungenreifeinduktion und aufgrund klinischer Progression wurde in der 32. SSW mit einer dosisreduzierten Chemotherapie nach ABVD-Schema (Adriamycin, Bleomycin, Vincristin und Dacarbazin) begonnen. In der 35. SSW wurde nach frustranem Einleitungsversuch die sekundäre Sectio caesarea mit einem gesunden Neugeborenen durchgeführt. Die Fortsetzung der Chemotherapie erfolgte am 10. Tag post partum in voller Dosis. Anhand der verfügbaren Literatur werden Besonderheiten eines Hodgkin-Lymphoms in der Schwangerschaft sowie diagnostische und therapeutische Optionen dargestellt.

Abstract

We report the case of a 24-year-old Gravida II/Para I who was diagnosed with Hodgkin’s disease (stage 2A) at week 28 of pregnancy. Due to clinical progression, a dose-reduced adriamycin, bleomycin, vincristin, dacarbazine chemotherapy was started at week 32. After unsuccessful induction of labor at week 35, secondary cesarean section with a healthy newborn was performed. Chemotherapy was continued with full dose 10 days postpartum. We discuss clinical management as well as diagnostic and therapeutic options with respect to the available literature.
