Massive recurrent synovial sarcoma of the hip Radiologic–histologic correlation in a case of failed magnet therapy

Abstract

We report the unusual case of a recurrent, massive synovial sarcoma of the hip, following magnet therapy failure.

Introduction

Synovial sarcomas are malignant soft tissue tumors that characteristically occur about major joints and are often associated with high mortality rates. Early recognition of this aggressive neoplastic process, as well as the extent of soft tissue invasion, can be achieved with MR imaging. We report an unusual case of a large recurrent synovial sarcoma of the hip, demonstrating the aggressive nature and growth potential of this tumor when conventional medical therapy is not performed.

Case report

The patient is a 20-year-old female who presented with a large mass at the anterolateral aspect of her right hip. She had prior resection of a right hip synovial sarcoma, over 10 years prior to this presentation. At the time of surgery, adjuvant chemotherapy was initiated, but the patient was lost to follow-up. Upon representation 10 years later, the patient reports that while undergoing magnet therapy in a neighboring nation, several months prior to presentation, she noticed a marked increase

Discussion

Synovial sarcoma is an uncommon high-grade mesenchymal malignancy of unknown etiology that affects males twice as often as females and is most prevalent between 15 and 35 years of age. This slow-growing tumor typically occurs near a joint, more commonly involving the lower, than the upper extremity [1]. The majority are deep-seated. Despite occurring near joints, rarely does the synovial sarcoma actually arise from intraarticular sources, such as synovium, bursa or tendon sheath. Histological
