    Scale-up of antiretroviral therapy (ART) has resulted in substantial reductions in HIV-related morbidity and mortality worldwide.1 With more people living with HIV receiving ART, and improved early detection of virological failure through better monitoring in resource-limited settings, a substantial number of patients might need to switch to second-line and subsequent regimens.2–5 Selection of optimal second-line ART has important clinical, public health, and programmatic implications.
