Abstract

Presacral cystic schwannomas are rare, difficult to diagnose, and have only been reported in women. We present the first such case in a male patient. It was incidentally diagnosed during an evaluation for renal colic in a 52-year-old man. The presacral lesion was cystic on computed tomography and measured 10 cm in diameter. The appearance on pelvic magnetic resonance imaging was hyperintense on T2-weighted images and of low to intermediate intensity on T1-weighted images. Complete excision was achieved without bladder injury. The patient voided without difficulty and was free of local recurrence at 12 months after surgery.

Case report

A 52-year-old man presented with symptoms of left renal colic. These were accompanied by long-standing constipation, perineal pressure, and a slow urinary stream and hesitancy. His past medical history included childhood bladder schistosomiasis, which resolved with medical management at the age of 12.

The physical findings consisted of a large, rubbery-like mass, situated anterior to the rectum. It displaced the prostate laterally and was intimately related to the left seminal vesicle.

Comment

Presacral schwannomas are rare tumors. The difficulty with their diagnosis and management relates to the nonspecific presentation and radiographic appearance. We report the first case of presacral cystic schwannomas in a man. In addition to schwannoma, the differential diagnosis of cystic presacral tumors includes mullerian ductal cyst, anterior sacral meningocele, lymphangioma, presacral epidermal cyst, and retroperitoneal malignant peripheral nerve sheath tumor. Two thirds of schwannomas have

Pelvic schwannomas are uncommon tumors that rarely present with urinary tract consequences. In fact, there is only one prior report of a pelvic schwannoma occurring in a male patient [1]. Furthermore, the occurrence of a presacral schwannoma after prior pelvic surgery has never been previously described.
