Renal transplantation: other problemsEffects of different calcineurin inhibitors on sex hormone levels in transplanted male patients
Section snippets
Materials and methods
We performed a comparative study of sexual hormone levels among cyclosporine- or tacrolimus-treated transplant male patients with stable renal function (creatinine clearance >30 mL/min). Patients with diabetes, severe hypertension, hyperparathyroidism, and over the age of 60 years were not included the study. Patients prescribed the following medications were excluded: H2 receptor blockers, spironolactone, ketaconazole, benzodiazepines, tricyclic antidepressants, and opiates. This cross
Results
There were no statistical differences in baseline levels of serum FSH, LH, TTE, and PRL between cyclosporine- and tacrolimus-treated male patients matched for age, mean duration of dialysis before transplantation, and duration of the posttransplant period. All the results were in normal ranges (Table 1).
Discussion
Endocrine abnormalities are common findings among end-stage renal failure patients. Sexual problems manifested by erectile dysfunction in men, menstrual abnormalities in women, and decreased libido and fertility in both genders have been well documented in chronic renal failure. Disturbances in the hypothalamopituitary-gonadal axis may responsible for these endocrine abnormalities. These circumstances have been attributed to decreased metabolic clearance of LH, FSH, PRL, and TTE. Although the

Sexual Dysfunction Among Patients With Chronic Kidney Disease
2021, Seminars in NephrologyCitation Excerpt :The typical immunosuppressive regimen after KT includes a calcineurin inhibitor (tacrolimus or cyclosporine), an antimetabolite (mycophenolate, azathioprine, or sirolimus), and/or low-dose corticosteroid (prednisone). Although calcineurin inhibitors impair spermatogenesis in animals,104,105 their effects on the human reproductive system are less clear,106,107 and they generally have not been associated with SD. Among antimetabolites, mycophenolate is the most frequent choice, and azathioprine typically is considered for those who do not tolerate mycophenolate.

For example, such studies would be able to determine whether calcineurin-inhibiting immunosuppressants, such as cyclosporine and FK506, affect reproductive functions of the recipients [22,23] as well as the host's immune response to the transplanted tissues. Orthotopic neonatal GFP ovary transplantation provides a powerful experimental rat model for studying ovarian development and the effects of environmental factors on adult ovarian function (Fig. 3).
