    The proposition of 7-year use of LNG-IUS 52 mg has been challenged due to the lack of clinical trials on contraceptive efficacy, without which it might be premature to initiate such a practice.188 Hence, clinical judgment and individual factors should be considered before advising off-label extended use of the LNG-IUS 52 mg, including access to and potential risks of IUC reinsertion.189 Given the reduced likelihood of spontaneous pregnancy in women over the age of 50 years, women who have the LNG-IUS 52 mg inserted at ≥ 45 years of age can use the device for 7 years (off-label use) (Table 5).170
