    The use of external radiation is another risk factor for the development of breast cancer,30 and this is particularly true in radiation therapy for childhood cancers, such as Hodgkin lymphoma.31 Their lifetime risk of breast cancer development can be comparable to that of BRCA positive mutations.32 Of note, the increased risk has been noted in any pediatric age of diagnosis and radiation therapy.33,34

    Prophylactic bilateral mastectomy is rarely discussed with HL survivors due to the lack of data in this regard (Boucher and Blaes, 2013). One might suggest that HL survivors had some special issues such as breast tissue changes related to radiation as well as emotional and physical effects of already experiencing cancer, making them more likely to accept this strategy unlike other high risk women (Boucher and Blaes, 2013). We are excitedly awaiting for the results of the trial assessing low-dose Tamoxifen for 2 years in HL patients with prior thoracic irradiation, which may offer a new avenue for prevention in this group (NCT01196936) (https://clinicaltrials.gov/ct2/show/NCT01196936).

    Bathia and other authors found the same [1,16,23,32], and it has recently [5] been reported that among HL patients who developed BC, 64% of all subsequent malignancies were carcinoma of the breast again. Some studies have suggested a risk similar to that of BRCA1 and BRCA2 mutation carriers [5,33]. Prophylactic mastectomy is effective in preventing the occurrence of BC in BRCA-positive women [34,35], but has no established role in cancer survivors as yet, and has rarely been discussed-though the high number of contralateral BCs seen in series like ours suggests that this topic warrants attention [33].
