Section snippets
Methods
All patients with a clinical diagnosis of traumatic anterior shoulder instability who were seen in a single unit (Liverpool Upper Limb Unit) between April 2000 and June 2003 were considered for inclusion. The inclusion criterion for participation in this study was a history of traumatic recurrent anterior shoulder dislocation. Exclusion criteria consisted of (1) single dislocation, and (2) previous shoulder stabilization surgery. Length of follow-up was 1.5 to 5 years (mean, 2.6 y).
Patients
Results
A total of 130 consecutive patients were included in the study, of whom 6 (5%) refused follow-up. Therefore, 124 patients (95%) completed the study. Age ranged from 17 to 49 years; mean (SD) age was 28 (7) years. The study group comprised 16 female and 108 male patients. The dominant arm was affected in 73 patients (59%) and the nondominant arm in 51 (41%). Characteristics of both groups are shown in Table 1.
At the time of surgery, 6 patients (5%) were found to have associated SLAP lesions.
Discussion
Arthroscopic Bankart repair has gained popularity over the past 10 years. Historically, arthroscopic repair for the treatment of patients with traumatic anterior instability has been less than satisfactory; this change is probably the result of evolution of the technique. First-generation techniques involved staple capsulorrhaphy, with high failure rates and complications linked to the metallic staples.16, 17, 18 Second-generation techniques included fixation with biodegradable tacks and
Conclusions
Arthroscopic anterior stabilization of the shoulder appears to have been effective: in this study; the overall failure rate was 9%, with 85% of patients returning to their preinjury levels of sporting activity. Both Panalok absorbable and G II nonabsorbable anchors appear to be effective, with no significant differences in outcome measures.
