Résumé

Introduction

La tumeur fibreuse solitaire de la plèvre (TFSP) est une tumeur rare. Son association avec l’hypoglycémie entre dans le cadre du syndrome de Doege et Potter.

Observation

Un homme âgé de 51 ans a été hospitalisé pour lipothymies liées à une hypoglycémie. L’examen clinique puis l’imagerie thoracique ont mis en évidence des anomalies correspondant à une masse occupant la presque totalité de l’hémithorax gauche. Après résection et étude histopathologique, une tumeur fibreuse pleurale a été diagnostiquée.

Discussion

La TFSP s’observe le plus souvent entre 50 et 70 ans. L’hypoglycémie est rare, de l’ordre de 4 % des cas. Elle s’explique par la sécrétion anormale d’un facteur de croissance insulin-like de type II. L’exérèse chirurgicale complète en constitue le traitement.

Introduction

Solitary fibrous tumor of the pleura (TFSP) is rare. When associated with hypoglycemia it is commonly referred to as Doege-Potter syndrome.

Case

A 51-year-old man was hospitalized for recurrent near-syncope associated with hypoglycemia. Clinical examination and then thoracic imaging clearly showed anomalies that corresponded to a mass occupying the entire left hemithorax. After resection, histological analysis diagnosed a fibrous tumor of the pleura.

Discussion

Solitary fibrous tumors of the pleura are observed most often in patients aged 50 and 70 years. Hypoglycemia is rare, occurring in approximately 4% of cases. It is explained by the abnormal secretion of a type-II insulin-like growth factor. This tumor is treated by complete surgical excision.
